Your GP as the central point of contact for your health
GP-centred care places your general practice at the centre of your medical care. Its purpose is to coordinate treatments more effectively, structure care processes and make your overall care safer and more personal. Your GP acts as your first point of contact and supports you with all health-related concerns.
Within this model, all further treatment steps, such as referrals to specialists or therapies, are coordinated in a targeted way. This helps us maintain an overview of diagnoses, medications and test results and avoid duplicate examinations or unnecessary strain. At the same time, the model enables continuous, trust-based care that is tailored to your individual state of health.
For you, this means clearly structured care, short coordination paths and a fixed medical contact person. If needed, we will advise you in detail on participation, the process and the benefits of GP-centred care and support you in making the decision that is right for you.
