Disease management programmes are long-term, structured treatment programmes for people with chronic conditions. Their aim is to provide your care continuously according to current medical guidelines, coordinate treatment processes more effectively and prevent possible complications or secondary illnesses at an early stage. At the same time, these programmes are intended to help improve your quality of life in a lasting way.
In our disease management programmes, we work closely with you: we plan regular follow-up appointments, review key health parameters and provide targeted consultations. This helps you understand your condition better, identify risks early and define individual treatment goals together with us.
Commonly offered disease management programmes include:
